MAYSLAKE VILLAGE SENIOR HOUSING
      Mayslake Village will be accepting applications on Thursday, October 2, 2008 for our
      Annex II, Wings II, Manor, and Villas one-bedroom apartments. Applications will be
      taken at the Oak Brook Hills Marriott Resort Hotel located at 3500 Midwest Road in
      Oak Brook, IL. We will be accepting applications for the Annex II, Wings II, Manor,
      and Villas buildings beginning at 9:00 am, and will close the waiting list at noon.

      Residency is restricted by age and income:

      Applicants must be 62 years of age, or older (if a married couple, at least one spouse must be 62 or older) and meet the following income criteria:

      Applicants for Annex II, Wings II, and Manor buildings must have a gross annual income of $26,400 or less for one person, or $30,150 or less for two persons. Rent is based on 30% of income.

      Applicants for the Villas must have a gross annual income of $42,200 or less for one person, or $48,250 or less for two persons. Rental amounts are currently $492 per month up to $559 per month.

      The apartments are one-bedroom units, with full kitchens, for independent seniors. Other admission requirements must be met. Location: Mayslake Village Campus

      The actual applicant(s) must be present at the time of application.
